Released in a US legacy sleeve printed in Japan with a predominantly brown and grey obi with a [l=Jazz-Forever Excellent] catalogue on the back. There is a 4 page insert with Japanese liner notes on page 1, [a=the modern jazz quartet] discography on pages 2 and 3 and page 4 is blank

Although the US legacy sleeve and Japanese obi state stereo this recording is mono
